2-1-1 San Diego has begun to remove barriers to healthcare through its Healthcare Navigation Program which guides clients through the complex healthcare system, extending 2-1-1 services beyond a simple information organization to become a single access point of health-related information and services. Although 2-1-1’s customary research and referral approach is useful in many situations, navigation is needed when cases are more complicated and specific to an individual’s particular circumstances.
2-1-1 San Diego’s Healthcare Navigator leads clients in accessing the help they need by performing an in-depth assessment of their situation, identifying health resources, assessing program eligibility, setting appointments at community clinics, assisting with prescription discount applications, locating transportation, and assisting in the completion and submission of Medi-Cal and Healthy Families applications.
The below reports detail the demographics and needs of clients calling 2-1-1 with health-related needs, as well as the applications completed and follow-ups made by 2-1-1 Client Service Representatives.
