About ACCESS and Transition Team Mental Health Services

Operates the ACCESS (Advocacy, Care, Choice, Empowerment, Success, Satisfaction) team, which provides psychiatric support services to approximately 100 of the highest utilizers of mental health fee-for-service inpatient care in San Diego County. Offers rehabilitation, teaches critical community living and survival skills, and provides support where people live, work, and socialize. Also provides medication support and, where appropriate, money management services through a representative payee program. Also operates the Transition Team, which provides intensive, short-term psychiatric case management services to adults. Services are designed to avoid repeated hospitalization by aiding in the stabilization of clients who need and desire assistance. In collaboration with the client and treatment professionals, services are provided for 30 to 60 days following an acute hospitalization or crisis residential admission.
