About Community Healthcare

Offers primary healthcare services and doctor appointments to all ages. Provides the following services: acute care; physical exams; sick visits; health maintenance; chronic disease management and education including care for diabetes, asthma & high blood pressure; immigration physicals; immunizations; flu shots; school, camp, sport and employment physicals; pediatrics including developmental assessment, well baby care, immunization, physical, CHDP exams, occupational and speech therapy; vision and hearing screenings; lead screening; TB screenings; pregnancy testing and counseling; family planning including all birth control methods and sterilization; screening and treatment for sexually transmitted infections; prenatal care and delivery including ultrasonography; nutrition assessment, counseling and education; health education; psychosocial assessment and counseling; childbirth preparation classes; gynecological services including breast and cervical cancer screening, colposcopy, cryosurgery, and LEEP procedures; internal medicine; dental; endocrinology; laboratory, rheumatology; optometry; ophthalmology; otolaryngology, ENT (Ear, nose, and throat) and Audiology services, pharmacy (limited pharmacy at all sites); podiatry; speech and hearing; HIV testing, counseling, primary care & case management. Also offers mental health services including counseling and psychiatry. Provides services to CMS-eligible clients. Not all services are available at all locations.
