About Hospice for the Homeless
Provides housing placement, case management, and support services to homeless and low income persons facing terminal illness/end of life issues through a partnership with the San Diego Hospice and Palliative Care. Individuals accepted into the program will have a case manager assigned to them. A personal plan of care will be developed by the patient"™s physician and San Diego Hospice team and may include hospice care, a physician/nurse, social worker, chaplain, home health aide, pharmacist, and bereavement counselor.
