About San Diego Health Care for the Homeless Project

Provides healthcare services to the homeless, including primary care, emergency dental care (extractions and temporary fillings), and mental health services including dual diagnosis assessment and counseling at multiple sites throughout Central and East San Diego County. Provides short-term case management services to homeless patients of the clinic. Vision Care Services. Case management services include bus tokens for medical appointments, assistance in completing forms, and referrals for emergency food, clothing, social services, and obtaining an ID. Free transportation to healthcare appointments at Family Health Center clinics is provided to homeless patients, Mental Health, Dental services, and Case Management at selected sites only; coordinate with the assigned case manager/social worker.
